CHRONOS

Questionnaire on Geologic Time and Earth History

Thank you for your interest in the CHRONOS questionnaire on geologic time and Earth history.

This questionnaire is designed for both teachers and students in grades 6-12. Please answer the following questions to the best of your ability. All information you provide will be kept completely confidential and will be used in summary form only.

Participation in the questionnaire is voluntary and anonymous. You can discontinue it at any time. It should take you about 20 minutes to complete.

The questionnaire will consist of two sections:

  1. Demographic information
  2. Science content questions

Scroll through each section to answer the questions. Click on the 'Continue' button at the end of each section to proceed, and click on the Final Submit button at the end of the questionnaire to submit your final responses.

If you agree to take the questionnaire and to contribute your responses to this study, please select the 'I agree' circle below.

If you have any questions about the study or have any difficulties with the online form, please contact Cinzia Cervato at cinzia at iastate.edu or by phone at 515-294-7583

If you wish to participate in the questionnaire, please select 'I agree' on the previous page.
